Details of our Wellness Membership
Wellness Membership: This is for patients NOT in the acute care phase. Studies continually show that in people who are regularly adjusted there is: more energy, better sleeping, less sickness, fewer hospital visits, fewer pharmaceuticals, and less likelihood to experience flare-ups from old injuries.
This is a 6-month program but will continue until you want to stop. It is auto-debited on the 5th of every month. The programs are “use it or lose it” and don’t carry over into the next month. These are non-insurance plans, none of these visits can be billed to insurance. You can use your HSA card.
Individual Wellness 1 Visit Per Month Plan:
1 chiropractic visit including therapies, electrical muscle stim/hot pk/roller.
- $55 per month = 1 chiropractic visit including therapies, electrical muscle stim/hot pk/roller.
Individual Wellness 2 Visits Per Month Plan: Adjustments only, no therapies.
- $70.00 per month = 2 chiropractic adjustments only per month.
- $85.00 per month = 2 chiropractic adjustments per month with therapy.
Individual Wellness 4 Visits Per Month Plan: Adjustments only, no therapies.
- $140.00 per month = 4 chiropractic adjustments only per month.
- $170.00 per month = 4 chiropractic adjustments per month with therapy.
Family Wellness 1 Visit Per Month Plan: Adjustments only, no therapies.
- For 2 or more family members = $35 per family member living in the same home.
Family Wellness 2 Visits Per Month Plan: Adjustments only, no therapies.
- For 2 or more family members = 2 chiropractic adjustments per month per person.
2 members= $136 3 members= $210 4 members=$280
Family Wellness 4 Visits Per Month Plan: Adjustments only, no therapies.
- For 2 or more family members = 4 chiropractic adjustments per month per person.
2 members= $280 3 members= $420 4 members=$560
WELLNESS MEMBERSHIP AUTO DEBIT
- Use it or lose it: If you miss your total amount of appointments that month, they do not accumulate into the next month. Most patients schedule their appointments out a month in advance, that way you get text reminders of your appointments so you don’t forget. We are very flexible with appointments; you can always move them as needed.
- Auto Debit: On the 5th of every month your account will be automatically debited. Please present the credit/debit/HSA card you want to use when signing up for this program.
- Discontinuing: You need to notify us 5 business days in advance before the auto debit for the next month if you wish to discontinue this program at the end of 6 months. You will need to send an email or type your discontinuation notice and present it to us. This is a Wellness Care Plan that will automatically continue after 6 months unless you let us know in writing you wish to discontinue through frontdesk@amesbarrychiro.com.
